This service evaluation is a study that aims to assess Macmillan’s Right by You study.This is an NHS England-funded intervention called Right by You Wessex, a model of integrated, personalised cancer care (https://wessexcanceralliance.nhs.uk/right-by-you-wessex/).This initiative was developed to ensure all people at all stages of their cancer journey are given care and support to address their needs. Core to the approach is the development of a personalised integrated care plan, co-produced with the patient and their care team across primary, secondary and community care. These plans are generated through the use of the CHAT & PLAN approach to supported conversations, an intervention that I developed during my time working at the University of Southampton.
